The considerable attention to financial interests in clinical research has focused mostly on academic medical centers, even though the majority of clinical research is conducted in community practice settings. To fill this gap, this article maps the practices and policies in 73 community hospitals and several hundred specialized facilities around the country for reviewing clinical investigators’ financial relationships with research sponsors. Community hospitals face a substantially different mix of issues than academic medical centers do because their physician researchers are usually (...) not employees, and more of their studies are later-phase, multicenter trials. Accordingly, community physicians stand to benefit personally from per capita payments, but they tend to have less direct influence over research findings when compared with their academic peers. These and other contrasts require separate guidance for community-based organizations that takes into account their particular needs, circumstances, and institutional capacities. (shrink)

Contractualism conceives of firm-stakeholder relations as cooperative schemes for mutual benefit. In essence, contractualism holds that these schemes, as well as the normative principles that guide and constrain them, are ultimately ratified by the consent and endorsement of those subject to them. This paper explores the empirical validity of a contractualist perspective on firm-stakeholder relations. It first develops a typology of firm-stakeholder contracting problems. It subsequently confronts this typology with empirical data collected in an interview study of concrete stakeholder management (...) practices, involving in-depth research interviews with forty-four managers working in the Dutch financial services industry. The findings of this theory-building study suggest that there are limits to the applicability of the contract model in the context of stakeholder management, and that disregarding either the model or its limitations may lead to highly ineffective firm-stakeholder relations. A continuoxis t- norm is a continuous map * from [0, 1]² into [0,1] such that is a commutative totally ordered monoid. Since the natural ordering on [0,1] is a complete lattice ordering, each continuous t-norm induces naturally a residuation → and becomes a commutative naturally ordered residuated monoid, also called a hoop. The variety of basic hoops is precisely the variety generated by all algebras, where * is a continuous t-norm. In this paper we investigate the structure of the (...) variety of basic hoops and some of its subvarieties. In particular we provide a complete description of the finite subdirectly irreducible basic hoops, and we show that the variety of basic hoops is generated as a quasivariety by its finite algebras. We extend these results to Hájek's BL-algebras, and we give an alternative proof of the fact that the variety of BL-algebras is generated by all algebras arising from continuous t-norms on [ 0,1] and their residua. The last part of the paper is devoted to the investigation of the subreducts of BL- algebras, of Gödel algebras and of product algebras. (shrink)

Background: In Southeast Asia, the process of obtaining informed consent is influenced by both culture and policy at the hospital and national level. Both physicians and nurses play vital roles in this process, but physicians influence the roles of nurses. Objectives: Since the physicians and nurses often have different perspectives, it is important to investigate their views about the informed consent process and nurses’ roles therein and whether there is a difference between ideal and experienced practice (reality), and whether this (...) differs across hospitals. Methods: A questionnaire was developed and a survey was conducted among physicians and nurses. Using exploratory factor analysis a three factor structure was determined: ‘nurses’ roles’, ‘barriers in informed consent’, and ‘adequacy of information’. Non-parametric tests were applied to compare nurses and physicians, and hospital setting. Participants and research context: Responses were obtained from 129 physicians and 616 nurses from two Indonesian hospitals. Those hospitals differ in ownership, location, and size. Ethical consideration: The study was reviewed by the hospital ethical committee. Participation was voluntary and confidentiality was ensured by keeping the responses anonymous. Findings: Physicians and nurses differ significantly on all three factors. The scores reflecting disparity between ideal and reality regarding nurses’ roles varied across professions, while barriers in informed consent differed between hospitals. Discussion: The differences between ideal and reality indicated that improvement in the informed consent process and nurses’ roles therein is called for. Varying views between physicians and nurses on nurses’ roles may hinder collaboration. The differences between hospital settings showed interventions may have to be customized for different settings. Conclusion: Views on nurses’ roles vary across professions. Views on barriers in informed consent vary across hospitals. Therefore interprofessional education is needed to promote interprofessional collaboration and intervention to improve informed consent practice should be tailored to the hospital context. (shrink)

In recent years, many philosophers of modern physics came to the conclusion that the problem of how objectivity is constituted (rather than merely given) can no longer be avoided, and therefore that a transcendental approach in the spirit of Kant is now philosophically relevant. The usual excuse for skipping this task is that the historical form given by Kant to transcendental epistemology has been challenged by Relativity and Quantum Physics. However, the true challenge is not to force modern physics into (...) a rigidly construed static version of Kant's philosophy, but to provide Kant's method with flexibility and generality. In this book, the top specialists of the field pin down the methodological core of transcendental epistemology that must be used in order to throw light on the foundations of modern physics. First, the basic tools Kant used for his transcendental reading of Newtonian Mechanics are examined, and then early transcendental approaches of Relativistic and Quantum Physics are revisited. Transcendental procedures are also applied to contemporary physics, and this renewed transcendental interpretation is finally compared with structural realism and constructive empiricism. The book will be of interest to scientists, historians and philosophers who are involved in the foundational problems of modern physics. (shrink)

This little volume shows a remarkable familiarity with both continental and Anglo-American literature on the subjects of probability and induction. Discussion is both expository and critical. The author argues in favor of a psychological-pragmatic interpretation of the principle of induction and the theory of confirmation.--A. P. D. M.

The place of scientific results, the role of scientific method, and the impact of the scientific temper in ethical theory are surveyed. The concept of an "existential perspective of an ethical theory"--"its view of the world and its properties, man's nature and condition, insofar as these enter into its understanding of moral processes and moral judgments"--is sketched and used to clarify the diversity of ethical approaches. M. Piguet believes that philosophy is, in a strict sense, a compromise between science and art. In the course of establishing this thesis he makes some interesting suggestions on the question of ordinary language. For him this idiom is the "culture language"; it embodies the cultural experience of an age; it can be neither technical nor colloquial; it is the language of the honnête homme. This is a book both sober and optimistic, both analytical and humane. In the course of a virtual reinstatement of a Socratic ethics, it is claimed that: analytic philosophy can decide moral issues, at least indirectly, by clarifying the relevant concepts; moral philosophers are wrong in saying that there can be no argument about generally accepted standards; our failure to resolve cases of ultimate ethical disagreement is only a measure of our ignorance of the human machine, and confusion about our (...) shared purposes; students of moral concepts cannot be neutral with respect to morals--analytic philosophers should not disguise their role as social reformers.--A. In this paper, we discuss an approach to ‘design for wow’ that focuses on the emotions that constitute a wow-experience. In this approach, the eliciting conditions of these emotions are used to define a product character with a high wow-impact. In addition to the approach, a measurable wow-index is introduced. First, a concept of wow is described in which wow is explained as a combination of fascination, pleasant surprise, and desire. The eliciting conditions of these three emotions are examined and (...) combined to a ‘wow-appraisal.’ This concept is applied in the design of a mobile telephone. A combination of qualitative and quantitative research methods was used to formulate concern themes relevant for the wow-appraisal. These themes were used to create a layered product character and a prototype of the final design. An evaluation study demonstrated that the product designed with this approach rated higher on the wow-index than alternative products. (shrink)

Objective To study how doctors care for their patients, both medically and as fellow humans, through observing their conduct in patient–doctor encounters. Design Qualitative study in which 101 videotaped consultations were observed and analysed using a Grounded Theory approach, generating explanatory categories through a hermeneutical analysis of the taped consultations. Setting A 500-bed general teaching hospital in Norway. Participants 71 doctors working in clinical non-psychiatric departments and their patients. Results The doctors were concerned about their patients' health and how their (...) medical knowledge could be of service. This medical focus often over-rode other important aspects of the consultations, especially existential elements. The doctors actively directed the focus away from their patients' existential concerns onto medical facts and rarely addressed the personal aspects of a patient's condition, treating them in a biomechanical manner. At the same time, however, the doctors attended to their patients with courteousness, displaying a polite and friendly attitude and emphasising the relationship between them. Conclusions The study suggests that the main failing of patient–doctor encounters is not a lack of courteous manners, but the moral offence patients experience when existential concerns are ignored. Improving doctors' social and communication skills cannot resolve this moral problem, which appears to be intrinsically bound to modern medical practice. Acknowledging this moral offence would, however, be the first step towards minimising the effects thereof. (shrink)

More than half of the book is devoted to what is almost a catalogue exposition of Western moral philosophy from Kant to Nowell-Smith. Although the author claims in the preface to have included "suitable references to Indian points of view, including the Buddhist," what references there are are usually too general, and never fully explained to be of any use to Western readers. Exposition tends to be disjointed, and the English leaves much to be desired. On the whole there is (...) much eclecticism and little originality.--A. P. D. M. (shrink)

As a clear, well documented and relatively complete introduction to Wittgenstein's famous book this commentary fulfills a long felt need. Rules for structuring Wittgenstein's statements are helpfully discussed, the terminology of the English translation is significantly improved upon, and the ontological purport of the Tractatus, as well as its striking parallels to Kantian philosophy, are convincingly stressed. But in recasting Wittgenstein's thought in a series of interpretative theses some basic themes become distorted: the notion of "category" is foreign to Wittgenstein's (...) thought and fails to capture the gradual transition from picturing to the "ladder"; the interpretation of "thing" as covering both objects and predicates or relations is highly dubious.--A. We are reviewing and summarizing evidence for the processes of acquisition of information outside of conscious awareness (processing information about covariations, nonconscious indirect and interactive inferences, self-perpetuation of procedural knowledge). A considerable amount of data indicates that as compared to consciously controlled cognition, the nonconscious information-acquisition processes are not only much faster but also structurally more sophisticated in the sense that they are capable of efficient processing of multidimensional and interactive relations between variables. Those mechanisms of nonconscious acquisition of information (...) provide a major channel for the development of procedural knowledge which is indispensable for such important aspects of cognitive functioning as encoding and interpretation of stimuli and the triggering emotional reactions. The relevance of nonlinear dynamics to calcium metabolism led us to reevaluate the role of Ca-regulating hormones in Ca homeostasis. We suggest that, firstly, the main Ca metabolic functions in rat-bone and gut - are organized as dynamic entities able to generate various temporal expressions, including self-oscillating patterns and, secondly, Ca homeostasis results from interaction between both metabolic and hormonal oscillators. Following this schema, a major role for the hormonal system, with its circadian pattern, could be to act directly on (...) metabolic functions or indirectly through feeding behaviour, in order to optimize, coordinate and synchronize the Ca fluxes at ECF level. Topics in the standard introductory logic course are all covered, with emphasis on the understanding of concepts rather than the development of skills in symbolic manipulation. Exposition is informal and rich in illustrations from reasoning in ordinary life. Exercises are also drawn from ordinary discourse, but they often call for the application of symbolic techniques. In combining accuracy and sophistication with simplicity and liveliness of style, the book is indeed a pedagogic success. --A. P. D. M.

Classical metaphysics from Plato to English empiricism arises out of the bad syntax and bad semantics of the Greek language. Phenomenology and existentialism arise out of similar excesses in German. The thesis, familiar to English readers, that all metaphysicians are duped by the poor logical form of ordinary language, is argued against an impressive command of the history of philosophy and with a degree of vehemence reminiscent of adolescent logical empiricism.--A. Meanings are construed by analogy with offices. But in contrast with similar views, offices are seen as "entities." In the final analysis all offices are reduced to that of stating facts. If the latter thesis were adequately defended the book would mark another reversal of philosophical thinking. But the author limits himself to reviving, restating, and modifying earlier positivist attempts to reduce all meaning to description.--A. P. D. M.

Meaning is interpreted in terms of distributional regularities in the corpus of a language; synonymy in terms of contrastive and non-contrastive sets of morphemic environments. For this blending of philosophical analysis and empirical linguistics, however, no defense is offered, though in the course of his sketch the author registers subtle and unconventional insights into key concepts and issues in philosophy of language.--A. This controversial book professes to be a "critical account of Linguistic Philosophy and a study of Ideology." It is clearly written and abounds with entertaining aphorisms. Philosophically important objections against the Oxford School are developed in the first hundred pages. The rest is highly repetitious, and, aside from rhetorical effects and remarks on the psychology and sociology of the movement, contribute little to a "refutation" of Linguistic Philosophy. As an alternative to Linguistic Philosophy, the book only vaguely hints to a (...) "clean," "non-oblique," and "non-insinuated" form of naturalism.--A. P. D. M. (shrink)
